Health Information

Please inform the Front Office and Health Room of any changes in your home address, phone numbers, and Emergency Contacts. This information is VERY important to the school and your child if he/she should suddenly becomes ill or injured.

 

If a child is ill, please keep him/her at home to lessen the spread of illness at school and call the office in the morning. Please check with our health aide or your child's doctor if you have any questions. Certain communicable diseases need medical clearance before re-entry to school.

 

Significant medical conditions should be indicated on the Emergency Card. If special conditions or physical restrictions are needed in school, a medical statement will need to be submitted yearly. We will work with you and our Public Health Nurse to determine if an Emergency Action Plan is needed.

 

Generally, children with food allergies or intolerances do not have a disability as defined under Section 504 or IDEA. If your child has a documented disability that restricts his or her diet, a Medical Diet Order form must be filled out by a medical doctor. The USDA does not recognize juice as a substitute for milk. 

 

Due to students with severe and sometimes life-threatening allergies, all parents and students are encouraged to strive for a peanut-free environment. Please be mindful of what you pack in your child's lunches and snacks. Check ingredients in pre-packaged products and do not send any food items to school containing peanuts, peanut products, or peanut traces.

 

According to Department of Health procedures, medication can only be administered in school with medical authorization. Please contact our health aide for a form SHS-36 and more information.

 

The DOH worked with HIDOE on new guidance for proper head lice management. The guidance is based on guidelines from the American Academy of Pediatrics, the Center for Disease Control and Prevention, and the National Association of School Nurses.

 

Ukus are sometimes confused with bedbugs, which they are not. Ukus do not transmit disease and should not be a reason for children to miss school. The school health practice for ukus include:

• Inspection by the school health aide of a student who may have ukus
• Notification of the parent/guardian that the student has ukus
• The student will return to class where they will remain until the end of the school day. Parents can choose to take their child home to begin treatment.

 

Some tips on managing ukus are:

• Check your child's hair at least once a week, and if ukus or nits (eggs) are found, treat promptly. You may call your child's physician for possible treatment solutions, some of which may be covered by insurance;
• Educate yourself and your child about ukus
• Treat ukus if they are found
• Consider keeping your child's hair slicked back or tied up to reduce the risk of acquiring them

Students are eligible for a home or hospital instruction if medical evidence shows that the student will not be able to attend school for more than 10 school days. Please contact the office if more information is needed.

 

By the first day of school, all new students must have Tuberculosis (TB) clearance and a completed Student Health Record (Form 14). Students must also have a physical examination and all required immunization. Students missing either of these requirements will not be permitted to enter school on the first day.
